Your privacy matters to us. The following explains, in everyday terms, how Pulse Medx Supply LLC (“PulseMedx”, “we”) collects and uses information when you visit pulsemedx.com, and what rights you have under U.S. and New York law.

Who we are

Pulse Medx Supply LLC is a supplier of durable medical equipment and medical supplies located at 33 Bond Lane, Hicksville, NY 11801. You can reach us by phone at (631) 342-5733. For the purposes of this policy, Pulse Medx Supply LLC is the business responsible for the personal information described here.

What this policy covers

This policy covers this website only. Information we receive while providing care, equipment or services is handled separately, as described below.

Health information is different. Protected health information that we create or receive as a health care supplier is governed by the federal HIPAA Privacy Rule and described in our Notice of Privacy Practices. Please do not send health information through this website.

Information we collect

Information you choose to give us

If you send us a message through the contact form, we collect what you type into it: name, email address, the type of inquiry, a subject and your message, plus your phone number and organization name if you choose to include them.

Information collected automatically

Like nearly every website, our web server automatically records standard technical information when pages are requested: your IP address, browser and device type (user agent), the pages requested, the date and time, and the referring web address. These server logs are used to keep the site running and secure and are kept for no more than 90 days.

The human check (CAPTCHA)

To protect our inbox and contact form from spam, we use a self-hosted “proof-of-work” check (ALTCHA). Your browser solves a small computational puzzle, and our own server verifies the result. It does not use cookies, does not track you, does not ask you to identify images, and does not send any information to a third party. Our server keeps a short-lived, one-way code of each verified check to stop it being reused, and a record of request counts from a scrambled (hashed) version of your IP address to prevent abuse; these records are deleted automatically.

What we do not collect

This website does not use cookies, analytics, advertising pixels, social media plug-ins, session-replay tools, or third-party fonts, maps or scripts. Nothing on this site loads content from other companies’ servers unless you choose to follow an outbound link (for example, “Open in Google Maps”).

How we use information

We use the information described above to:

  • read and respond to your inquiry, and follow up by your preferred contact method;
  • operate, maintain and secure this website, including detecting and preventing spam, fraud and abuse;
  • keep ordinary business records of correspondence; and
  • comply with legal obligations and respond to lawful requests.

We do not use your information for advertising, and we do not build profiles about you.

How we share information

We share personal information only as needed to run this website and our business, specifically:

  • Service providers — our website hosting and email providers, who process information on our behalf and only according to our instructions;
  • Legal requirements — when required by law, subpoena or court order, or to protect the rights, property or safety of our patients, customers, staff or others;
  • Business transfers — in connection with a merger, sale or transfer of all or part of our business, subject to this policy.

We do not sell personal information, and we do not share personal information for cross-context behavioral advertising or targeted advertising. We have not done so in the past 12 months.

Do Not Track and Global Privacy Control

Because this website does not track visitors across other websites, browser “Do Not Track” signals do not change how the site works. Where state law treats a Global Privacy Control (GPC) signal as a request to opt out of the sale or sharing of personal information or targeted advertising, we honor it — although, as stated above, we do not engage in those activities.

How long we keep information

  • Contact form messages: kept in our business email only as long as needed to respond and to maintain ordinary business records, then deleted according to our record-keeping practices.
  • Anti-spam records: one-way codes and hashed request counters are deleted automatically, generally within 24 hours and never later than 90 days.
  • Web server logs and contact-form delivery logs (which record a reference number and outcome, not your message): up to 90 days.

How we protect information

We use reasonable administrative, technical and physical safeguards, including encryption in transit (HTTPS), restricted server access, keeping secrets outside the public website, and collecting as little information as possible. No method of transmission or storage is completely secure, so we cannot guarantee absolute security.

As required by the New York SHIELD Act (N.Y. Gen. Bus. Law § 899-bb), we maintain reasonable safeguards to protect the security and confidentiality of private information, and we will notify affected individuals and authorities of a security breach as required by N.Y. Gen. Bus. Law § 899-aa.

Children’s privacy

This website is intended for adults and is not directed to children under 13. We do not knowingly collect personal information from children under 13 through this website. If you believe a child has sent us personal information, please contact us and we will delete it. Parents and guardians may contact us by phone about a child’s care or needs.

Your privacy choices and rights

Wherever you live in the United States, you may ask us to tell you what personal information we hold about you from this website, to correct it, or to delete it. We will respond within the time required by applicable law (and in any case within 45 days), and we will not discriminate against you for making a request. We may need to verify your identity before acting on a request — usually by confirming details of your message or by phone. You may use an authorized agent where state law allows; we may ask the agent for proof of authorization.

New York residents

New York does not currently have a comprehensive consumer privacy law like those in some other states, but we extend the access, correction and deletion choices described above to New York residents. Our security and breach-notification commitments under the New York SHIELD Act are described in “How we protect information.”

Residents of other states

If you live in a state with its own consumer privacy law, you may have additional rights. Contact us and we will honor them as that law requires.

How to make a request or ask a question

You can contact us about this policy or make a privacy request by any of these methods:
