About this notice

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Effective date: November 1, 2026

This Notice of Privacy Practices applies to Pulse Medx Supply LLC (“PulseMedx,” “we,” “us”), a supplier of durable medical equipment and medical supplies, located at 33 Bond Lane, Hicksville, NY 11801. If you have questions, please contact our Privacy Officer at (631) 342-5733.

Our responsibilities

  • We are required by law to maintain the privacy and security of your protected health information (“PHI”).
  • We will let you know promptly if a breach occurs that may have compromised the privacy or security of your information.
  • We must follow the duties and privacy practices described in this notice and give you a copy of it.
  • We will not use or share your information other than as described here unless you tell us we can in writing. If you tell us we can, you may change your mind at any time by letting us know in writing.

How we use and share your health information

Treatment

We can use your health information and share it with other professionals who are treating you. For example, we use and share your information to fill a physician’s order for equipment or supplies, to coordinate delivery, set-up and instruction, and to communicate with your doctor about what you need.

Payment

We can use and share your health information to bill and get payment from health plans or other entities. For example, we may give your health plan information about the equipment you received so it will pay us, or ask it to confirm coverage before we provide an item.

Health care operations

We can use and share your health information to run our organization, improve your care and contact you when necessary. For example, we may use your information to review the quality of our service, train staff, and manage resupply schedules.

Business associates

We may share information with companies that perform services for us (for example, billing or information-technology vendors). They must sign agreements to protect your information.

Appointment and service reminders

We may contact you with reminders or information about your care or the services you receive from us.

Other ways we may use or share your information

We are allowed or required to share your information in other ways — usually in ways that contribute to the public good, such as public health and research. We must meet many conditions in the law before we can share your information for these purposes:

  • Public health and safety — preventing disease, reporting adverse reactions to medications or product problems, reporting suspected abuse, neglect or domestic violence, and preventing or reducing a serious threat to anyone’s health or safety.
  • Research — under conditions required by law.
  • As required by law — including to the U.S. Department of Health and Human Services if it wants to confirm that we are complying with federal privacy law.
  • Organ and tissue donation requests, and to medical examiners, coroners or funeral directors when an individual dies.
  • Workers’ compensation, law enforcement and other government requests — for workers’ compensation claims; for law enforcement purposes or with a law enforcement official; with health oversight agencies for activities authorized by law; and for special government functions such as military, national security and presidential protective services.
  • Lawsuits and legal actions — in response to a court or administrative order, or in response to a subpoena.
  • Family and friends involved in your care — with your permission, or if you are unable to tell us your preference (for example, if you are unconscious), when we believe it is in your best interest.

Uses that require your written permission

We will never share your information for the following unless you give us written permission (an “authorization”): marketing purposes; sale of your information; and most sharing of psychotherapy notes. Other uses and disclosures not described in this notice will be made only with your written authorization, which you may revoke in writing at any time (except to the extent we have already acted on it).

If we receive records that are protected by federal confidentiality rules for substance use disorder treatment records (42 CFR Part 2), we will not use or disclose those records in civil, criminal, administrative or legislative proceedings against you unless you give written consent or a court order is obtained, as the law requires.

Your rights

When it comes to your health information, you have certain rights:

  • Get a copy of your health and claims records. You can ask to see or get an electronic or paper copy of your records. We will provide a copy or a summary, usually within 30 days of your request. We may charge a reasonable, cost-based fee.
  • Ask us to correct your record. You can ask us to correct information you think is incorrect or incomplete. We may say “no,” but we will tell you why in writing within 60 days.
  • Request confidential communications. You can ask us to contact you in a specific way (for example, a particular phone number) or to send mail to a different address. We will say “yes” to all reasonable requests.
  • Ask us to limit what we use or share. You can ask us not to use or share certain information for treatment, payment or our operations. We are not required to agree, and we may say “no” if it would affect your care. If you pay for a service or item out-of-pocket in full, you can ask us not to share that information with your health insurer for payment or operations, and we will say “yes” unless a law requires us to share it.
  • Get a list of those with whom we’ve shared information. You can ask for a list (accounting) of the times we’ve shared your information for six years prior to the date you ask, who we shared it with, and why. We will include all disclosures except those about treatment, payment and operations and certain others (such as any you asked us to make). We will provide one accounting a year for free.
  • Get a copy of this privacy notice. You can ask for a paper copy at any time, even if you agreed to receive it electronically.
  • Choose someone to act for you. If you have given someone medical power of attorney or if someone is your legal guardian, that person can exercise your rights and make choices about your health information. We will make sure the person has this authority before we take any action.
  • File a complaint if you feel your rights are violated (see below).

Your choices

For certain health information, you can tell us your choices about what we share. You have both the right and choice to tell us to share information with your family, close friends or others involved in your care, or to share information in a disaster relief situation. If you are not able to tell us your preference, we may go ahead and share your information if we believe it is in your best interest, or when needed to lessen a serious and imminent threat to health or safety.

State law

New York law, including the Public Health Law and provisions protecting HIV-related, mental health and genetic information, may give you additional protections. Where New York law is more protective than HIPAA, we follow New York law.

Complaints

You can complain if you feel we have violated your rights by contacting our Privacy Officer: Pulse Medx Supply LLC, Attn: Privacy Officer, 33 Bond Lane, Hicksville, NY 11801; telephone (631) 342-5733.

You can also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights by sending a letter to 200 Independence Avenue, S.W., Washington, D.C. 20201, calling 1-877-696-6775, or visiting www.hhs.gov/ocr/complaints.

We will not retaliate against you for filing a complaint.
